Completing the Form

Employees need to ensure information provided is accurate and answer questions correctly, as workflows will direct the form to the appropriate offices for approvals or denials.

Questions employees will encounter:

  • Are you requesting enrollment after the 5th day of the term (Monday, January 25, 2021)?
  • Do you meet the pre-requisites for the course?
  • Is department consent is required for enrollment?
  • Is the class is full?
  • Does a time conflict exist with another class?
  • Are you requesting to take a course as audit? 
  • Are you requesting to audit only and not enroll at WSU? Upload the Admission to Take A Class Audit form.
