Employees need to ensure information provided is accurate and answer questions correctly, as workflows will direct the form to the appropriate offices for approvals or denials.

Questions employees will encounter:

  • Are you requesting enrollment after the 5th day of the term?
  • Do you meet the pre-requisites for the course?
  • Is department consent is required for enrollment?
  • Is the class is full?
  • Does a time conflict exist with another class?
  • Are you requesting to take a course as audit?
